During our lifetime we all experience transitions; times when we journey from one stage to another. These transitions range from chronic illnesses to unexpected health concerns to the normal processes of aging. Geriatric Care Managers can help guide families through these transitions.

We are an association of private practitioners whose purpose is the development, advancement and promotion of humane and dignified social, psychological, and health care for the elderly and their families. The Midwest GCM Conference will be held in a unique Midwestern setting, off the beaten path. Don’t fret about transportation; book your flight into Detroit Metro or Toledo Express and there will be private vans providing rides to the conference site.
