During our lifetime we all experience transitions; times when we journey
from one stage to another. These transitions range from chronic
illnesses to unexpected health concerns to the normal processes of aging. Geriatric Care Managers can help guide families through these transitions.
We are the Midwest Chapter of the National Association of Professional Geriatric Care Managers. Members conduct their practices within the States of Illinois, Indiana, Iowa, Kansas, Kentucky, Michigan, Minnesota, Missouri, Nebraska,
North Dakota, Ohio, South Dakota and Wisconsin.
We
are an association of private practitioners whose purpose is the development, advancement and promotion of humane and dignified social, psychological, and health care for the elderly and their families. The association is committed to working towards the highest quality of care for the elderly and their families through education, advocacy, and high standards of practice.
